- Summer Staff Application Form - You will need Adobe Reader 8 or later to complete this paperless form (download for free here). Please fill out the application completely and digitally sign the document last. You will not be able to change/complete the form once it is signed. Save the completed form with your name in this format: JohnDoeApp2012. Attach the application to an email, along with your personal testimony and a photo, and send to summerstaff@gulllake.org.
